Dr. Marc Brand describes the symptoms of external hemorrhoids.

Dr. Brand:
External hemorrhoids and their symptoms relate to them being belonging external and having a skin covering. That skin covering is very, very sensitive, but it is also very protective. So the major problem external hemorrhoids experience is a blood clot that forms within them, and because of that blood clot and the swelling that follows, that will cause a great deal of exquisite discomfort.

It’s typically a constant pain that lasts somewhere in the neighborhood of three to five days, along with the swelling, and then swelling and discomfort will start to dissipate if the hemorrhoid in the skin over it ruptures, then the clot will come out. The swelling and pain will decrease more quickly. If the body has to dissolve the blood clot, then that may take a longer period of time.
