I am currently trying to concieve. I had my IUD removed Jan 21, 2010. The first day of my last period was feb 18th. I have had sex a few times since then. Could I be pregnant already and not know it? I won't have a missed period until the end of March. How can I tell before then or is it even likely that I am pregnant? I have taken ovulation tests and had sex within the time I was "ovulating". Please let me know what you think.

Hey dianne, ive been on birth control for about a year, i got my period on a wednesday and it ended around sunday/monday. me and my boyfriend had unprotected sex on tuesday the 2nd wednesday 3rd and friday 5th. He didnt ejaculate inside of me (i know pull out isnt safe) but right now its the 10th and im feeling cramps. they started last night and they felt worse than period cramps, but today they feel alot lighter. Do you think this could be implantation pain? i've heard that ovulation pain can occur but because im on the pill i dont see how thats possible. i take my pill promptly at the same time every day within a few minutes. Is it possible i am pregnant?

Hi, Anon,

If you are using birth control and you didn't miss any of your pills, you should be fine. Especially since you take them promptly every day. But very very occasionally women do get pregnant on the pill (when they say it is 97-99 percent effective, that still means that 1 to 3 women out of every 100 can get pregnant). So let's do the math and see what we can figure out.

If you got your period on a Wednesday, that is day one. of your cycle
So you had unprotected sex on days 7, 8 and 10 of your cycle. And now it's a few days later and you're feeling crampy.

Honestly, I don't think this would be pregnancy. Implantation cramping/bleeding is usually quite light, and the bleeding is like spotting. What you are describing seems a bit more severe, more like early period cramping. (And that can simply be due to hormonal fluctuation and stress).

Unfortunately, the only way to know for sure is to get closer to a missed period. And since you don't give dates for everything in your question, I'm unsure. When is your next period due?

Hi,
I just wanted to add to the great information Diane gave you.

You did not mention if you missed any birth control pills, just that you are having some unusual symptoms.

If you did not miss any birth control pills, then you actually had PROTECTED sex with your boyfriend (against pregnancy, not against STDs). Unprotected sex means sex without use of any type of birth control (pill, ring, condom, IUD, etc).

The pill, when used consistently and correctly, is about 99% effective at preventing pregnancy (without a condom).

You are right--women who use the pill are protected against pregnancy because it works by:
- preventing ovulation
- changing uterine lining (making it less inhabitable by fertilized egg)
- changing cervical mucus (making it more difficult for sperm to travel into uterus)

A woman can conceivable ovulate while taking the pill, that is why it is not 100% effective, but at 99% effectiveness rate...this is very rare.

Lastly, if you are expecting your "period" (women do not have periods while on the pill, when you bleed during your inactive pills it is actually "withdrawal bleeding". Women need to ovulate in order to have a period) in two weeks, than any symptoms you are experiencing now are likely unrelated to a pregnancy. You are probably experiencing "breakthrough bleeding" which is any bleeding in-between your expected period (or withdrawal bleeding) days, and this is common when taking hormonal contraception.

does this help?

Hi, I've been on the pill for a little over a year now and I think I might b pregnant. I got my period last month and I'm not supposed to get if for another week. But I've had a period like thing for the past week. This is very abnormal because I always get my period on Wednesday and only for 5 days. That's how it's been for the past year so I'm really scared as to why I'd get one now. And I have been really tired lately. And I kno this is stupid but me and my boyfriend never have protected sex. Could I be pregnant? I'm really scared so anything you could tell me would be great.

Anon,

I am pretty sure you're not pregnant.

First of all, you ARE having protected sex. Birth control pills are just that -- birth control protection. They do not protect you from STDs, of course; only something like a condom can do that. But the pill does protect you from a pregnancy.

And second, if you WERE pregnant, you wouldn't be having periods at all. When you have a period, your uterus is getting rid of the tissue it built up to nourish a fertilized egg if one implanted itself. When that doesn't happen, you bleed and have a period. So since you are having periods, that's another sign that you're not pregnant.

What may actually be happening, though, is breakthrough bleeding. Sometimes -- especially when we've taken one kind of birth control pill for a while -- we get bleeding that comes when we aren't supposed to be having a period, and that's called breakthrough bleeding. It can happen for no apparent reason. It can be a change in diet or exercise, a sickness (have you been ill in the last couple of months? Did you take an antibiotic?) or an increase in stress. Our hormones are touchy little things, and something like stress can really get them out of whack.

If I were you I would simply call your doctor's office and ask to speak to the nurse, the nurse practitioner or the physician's assistant. Tell her or him that you think your birth control prescription may need to be looked at because you've had breakthrough bleeding the last couple of months. It may be that you just need a slightly different prescription and you'll go back to being your regular self.

Does that help? Will you come back and update us?
