My mom has smoked for 50 some years, she has small cell lung cancer and it has matastasized to her brain now, she had 5 radiation treatments week ago, and wants a cigarette. It has also spread to her liver, bones and blood

Addiction has caused your mom to be terminally ill - does she have any chance of survival?

We can't say what will happen if she has a cigarette - it won't kill her on the spot and at this point it probably doesn't matter anymore. It won't make her feel any better though, even if she thinks it will.
