Having a pacemaker fitted should not cause weight gain.
Exercising an hour a day is excellent, as long as you are feeling well. But extra caloric intake is the main reason for people to gain weight unless medication is causing it. Have you been put on heart medication? If so. check to see if weight gain is a side effect. Otherwise make sure you are eating healthily.
If you continue to gain weight, talk to your doctor to have your hormone levels checked.
