Unfortunately it is often overlooked among our kids. It can happen from trauma (including orthodontic movement). It probably should have been noticed but I am glad you were attentive enough to see it yourself. Sometimes the trauma can show up down the road and not immediately.
If the tooth eventually had a root canal you can do what is called internal bleaching to whiten it to better match the surrounding teeth.
