If the doctor told your friend not to run....I would take the doctor's advise. I will assume that the doctor already knows about your friend's running history and his marathon intentions.
Unfortunately, to go against doctor instructions is putting your friend at risk for possible issues later. I hope that your friend listens to his doctor.
