My ex-brother-in-law has a son who had to wear diapers even at the age of 10 because he had bed-wetting problems. He's in high school now, and I have no idea if he still has this issue, but I really believed that it was more of a mental/emotional issue. He seemed like a pretty stressed-out kid as his parents had a lot of high-end academic and sports-related expectations for him.
