Dr. Shaw describes how breast augmentations are performed.

Dr. Shaw:
Breast augmentations are done by multiple means. The most popular in my practice is the transaxillary approach, putting in the implant through a small incision of the arm, whether it’s saline or silicone, and I usually put that under the muscle. There’s over the muscle and under the muscle implants and then there’s a third way and that’s the way that’s becoming most popular, one that we have been doing for many years.

Most of the time, what I will do within a breast implant is put it, the implant in here, it will go under the muscle on the top. There’s a release of the lower portion of the muscle so that, if you looked at this diagram the upper part of the implant is under the muscle and the lower part of the implant is over the muscle and I like to call that biplanar and the biplanar augmentation gives you a much more natural look because the muscle will now push on top of the implant allowing the bottom of the implant to give you a nice, round shape, giving you a more natural, tear-dropped breast augmentation.

I prefer to go through the axillary route, others might go through, or I might, if the patient elects, to go through the inframammary route or around the areola.
